Don’t Forget to File National Health Insurance Notification Forms As Necessary

National Health Insurance is a program in which everyone helps each other in maintaining good health. If any of the conditions described on the right apply to you, you must file a notification form within 14 days. Please complete the designated procedures at the NHI and Pension Section.
Insurance premiums must be paid from the date of eligibility. If you are late in filing your enrollment form, you will be charged for premiums dating back to the date of eligibility (a maximum of two years). However, all medical expenses paid before filing the appropriate forms must be personally borne. In addition, if you delay in filing your withdrawal form, your date of loss of eligibility for National Health Insurance will be dated back to the day you enrolled in the health insurance plan at your place of employment. If you use your National Health Insurance card at a medical institution for treatment or prescriptions after losing eligibility, you must pay back all medical expenses covered by National Health Insurance at a later date.
Please note that those who work in a company should enroll in the pension and health insurance schemes at their place of employment and are not eligible to enroll in the National Health Insurance of the city office.

If you leave Japan Health insurance card, documentation showing your scheduled date of departure (such as your plane ticket)
If a member dies Health insurance card, bankbook and name stamp of the chief mourner, receipt from a funeral company
Other If there is a change in your address, name or the head of the household
Health insurance card
If your household has merged or separated
If your health insurance card is lost or torn Foreign resident registration card

*If there is a change in the head of the household, please bring the health insurance card of the entire household. The head of the household indicated on the card must be changed.
When foreign residents enroll, there are designated requirements concerning the visa type and period of stay. Please call for more information.
There is a program by which you can choose to continue health insurance membership for those who are planning to retire from work. Please ask the coordinator at your place of employment for more information.
