Note: The cost for the following households is just ¥2,000:
(1) Households with an elderly person (65 years of age or older)
(2) Households with a disabled person: A copy of his/her disability handbook is required
(3) Low-income households: A copy of the certificate of resident's tax exemption of the head of the household is required
・ Applications: Apply by mail or fax to the Disaster Prevention Section (Shinagawa City Office, 140-8715; Tel: 5742-6695; Fax: 3777-1181) giving your name, address, date of birth, age, telephone number, number of radios desired, category (such as general, household with an elderly/disabled person, or low-income household) and required documents.
Note: The radios will be delivered in mid-October or later.
