National Health Insurance Premiums for FY2015 Have Been Determined

National Health Insurance premiums for April 2015 through March 2016 are the sum of the basic portion (for medical treatment), the latter-term elderly support portion, and the Nursing Care Insurance premiums (for those between 40 and 64 years of age).
The per capita amount and insurance premium rates for income-based amounts are determined based on the amount of medical benefits covering medical expenses, the support portion for Latter-Term Elderly Medical Insurance, premiums paid by Type 2 members of Nursing Care Insurance, and other factors.

○ Calculation Procedures

Annual Insurance Premium for a Household = (1) Basic Portion (for Medical Treatment) + (2) Latter-Term Elderly Support Portion + (3) Nursing Care Insurance Premiums

(1) Basic portion (for medical treatment; maximum ceiling: ¥520,000) = Incomebased amount (Standard leviable income* of all household insurance members × 6.45%)+Per-capita amount (¥33,900 × No. of insurance members in the household)
(2) Latter-term elderly support portion (maximum ceiling: ¥170,000) = Incomebased amount (Standard leviable income* of all household insurance members × 1.98%)+Per-capita amount (¥10,800 × No. of insurance members in the household)
(3) Nursing Care Insurance premiums (maximum ceiling: ¥160,000) = Incomebased amount (Standard leviable income* of all household insurance members between 40 and 64 years of age × 1.34%) +Per-capita amount (¥14,700 × No. of insurance members in the household between 40 and 64 years of age)
*Standard leviable income = Total income and so on - ¥330,000 (basic deduction)

○ FY2015 Payment Months and Calculation Period

The premiums for the year will be divided into 10 installments to be paid from June through March. No payments are made in April or May.
Note:Invoices are mailed in June (for June through October) and November (for November through March).
Please note that households that pay via account transfer will only receive notification of the premium amounts.

  • If you have not reported income, please do so for the municipality in which you resided as of January 1. Insurance premiums cannot be reduced unless each and every member of the household files an income report, even if the incomes of all household members are low.
  • If you moved into Shinagawa City on or after January 2, the City Office will contact the government of the municipality where you lived previously about your income. At first, you will be notified of the per capita amount only. Once your income has been confirmed, you will then be sent a notification of adjusted premiums.

Insurance premiums will be deducted from the head of household’s pension payments (special collection) between 65 and 74 years of age when all of the following apply.

(1) The head of household is a National Health Insurance member
(2) All the National Health Insurance members in the household are between 65 and 74 years of age
(3) The annual pension amount eligible for special collection is ¥180,000 or more, and the sum of National Health Insurance premiums and Nursing Care Insurance premiums does not exceed half of the pension amount

Note: FY2015 premiums have been fixed for persons who already have National Health Insurance premiums deducted by pension payments; premiums, adjusted for April and June amounts, will be made from August onward. Persons who wish (apply) to pay their premiums by automatic debit from a bank account can have their premium payments handled as ordinary collection rather than as a deduction from their pension payments.
