How to Pick Up a “My Number” Card:Upon receiving an Issuance Notification and a Booking Notification from Shinagawa City, after making a booking to pick up their "My Number" Card either through the Shinagawa My Number Call Center or through the bookings form on the especially-established website, individuals shall be able to come and pick up their "My Number" Card from the location indicated on the Issuance Notification that was sent to them. When doing so, they are asked to have in their possession the following documents: the Issuance Notification, their "Notification" Card and proof of identity (a driver's license, a passport, a basic resident registration card, etc.). ● Application procedures at Shinagawa City offices to change from January From January, there will be a number of changes made to the procedures by which certain applications are to be completed at Shinagawa City offices. • When completing social security application forms and documentation related to the filing of tax returns, etc., it shall be necessary for applicants to include their individual number.
