Information about My Number

●Have You Received Your “My Number” Notification Card?

You will need your notification card or My Number Card (Individual Number Card) for social insurance-related matters, tax applications and other documents. Please inquire if you have not yet received your notification card.

Note:If your workplace requires you to show your notification card on short notice, it is possible for the city to provide a copy of your residence record printed with your Individual Number (an issuing fee is required).

●How to Have Your My Number Card (Individual Number Card) Issued

My Number Card is a publically recognized form of identification that allows you to submit documents such as income tax returns and tax payment system documents electronically. You can apply for your Individual Number Card one of the following ways.

1. Fill out the application form included with your notification card and send it by mail using the provided return envelope.
2. Scan the barcode printed on the application form using your smartphone or computer to apply electronically.
3. Bring the application form to the photo booth on the 3rd floor of the Shinagawa City Office Daini Chosha. This machine (fee required) has a My Number Card application function.

Once your application is received, you will receive an Issuance Notification and a Booking Notification from Shinagawa City by post. Make a reservation to pick up your card through Shinagawa My Number Call Center or the website especially established for this purpose. When you go to pick up your Individual Number Card at the indicated location, please bring your Issuance Notification, your My Number Notification Card, and a piece of identification (driver’s license, passport, Basic resident registration card, etc.)
