National Health Insurance Premiums for FY2018 Have Been Determined

National Health Insurance premiums for April 2018 through March 2019 are the sum of the medical treatment portion, the latter-term elderly support portion, and the Nursing Care Insurance premiums (for those between 40 and 64 years of age).

● FY2018 Payment Months and Calculation Period
The premiums for FY2018 (for April 2018–March 2019 payments) will be announced in mid-June, and will be divided into 10 installments to be paid from June through March 2019. No payments are made in April or May.
- Mid-June: Insurance premiums notification and 6 invoices (for June–October payments and a one-time advance payment for the entire year) are mailed out.
Note: Households that pay via account debit will only receive notification of the premium amounts.
- For households that pay premiums using an invoice: 5 invoices (for November 2018–March 2019 payments) will be sent in the middle of November.

● If you have not reported income for FY2017, please do so for the municipality in which you resided as of January 1, 2018. Insurance premiums cannot be reduced unless each and every member of the household files an income report, even if the incomes of all household members are low.

● If you moved into Shinagawa City on or after January 2, 2018, the city office will contact the government of the municipality where you lived previously about your income for FY2017. At first, you will be notified of the per capita amount only. Once your income has been confirmed, we will recalculate your premiums and send you a notification of adjusted premiums.

● If you have lost your job due to dismissal or the termination of your employment contract, you must apply to have your premiums reduced. Please bring an employment insurance qualification card that states the reason of the loss of job is involuntary retirement.

● If you are the head of your household, between 65 and 74 years of age, and all the following apply to you, your premiums will be collected from your pension (special collection). However, if you wish to pay by account debit, you can choose to reject this special collection method.
1. The head of household is a National Health Insurance member
2. All the National Health Insurance members in the household are between 65 and 74 years of age
3. The head of the household’s annual pension amount is 180,000 yen or more, and the household’s total of National Health Insurance premiums and Nursing Care Insurance premiums does not exceed half of the pension amount

● Calculation Procedures

Annual insurance premium for a household
Medical treatment portion (maximum ceiling: 580,000 yen) =
Income-based amount (standard leviable income* of all household insurance members × 7.32%) +
Per-capita amount (39,000 yen × No. of insurance members in the household)
Latter-term elderly support portion (maximum ceiling: 190,000 yen) =
Income-based amount (standard leviable income* of all household insurance members × 2.22%)+
Per-capita amount (12,000 yen × No. of insurance members in the household)
Nursing Care Insurance premiums (maximum ceiling: 160,000 yen) =
Income-based amount (standard leviable income* of all household insurance members between 40 and 64 years of age × 1.51%) + Per-capita amount (15,600 yen × No. of insurance members in the household between 40 and 64 years of age)

* Standard leviable income = Total income and so on − 330,000 yen (basic deduction)
