An Announcement concerning COVID-19 Vaccinations
COVID-19 vaccination tickets were mailed out to residents 65 years of age and older
The information below is current as of May 21.
When you receive your vaccination ticket packet, please be sure to read the information inside.
Notes:
1. The standard date for age is as of March 31, 2022.
2. Vaccinations are free of charge, and available for anyone who wishes to receive them.
3. Tickets will also be sent to residents 64 years of age and younger, depending on supplies of the vaccine.

To residents currently undergoing treatment (e.g., taking medication)
Please be sure to ask the doctor treating your illness beforehand to find out whether you can be vaccinated.
Vaccination flow of procedures
❶ Vaccination tickets are delivered |

❷ Please check the vaccination schedule and make reservations for your first shot
(the reservation number on your vaccination ticket is required)
● Residents 75 years of age or older are already able to make reservations.
● Residents 65 years of age or older can make reservations from June 14 (Mon.).
Two methods for making reservations
(1) Telephone reservations: Call the Shinagawa Coronavirus Vaccination Call Center
(2) Web reservations: Complete the Shinagawa coronavirus vaccination online reservation form
Note: Please check the Shinagawa City website for more information on reservation procedures and vaccination locations in the city.

❸ Go to the vaccination location or medical institution you selected and receive your vaccination
Notes:
1. Please bring your vaccination ticket, pre-checkup slip (please complete the slip in advance) and official documentation to confirm your ID, such as a residence card, driver’s license or individual number (My Number) card.
2. You cannot be vaccinated unless you bring your vaccination ticket and official documentation to confirm your ID. |

❹ Make a reservation for the second shot of your vaccination
Note: You will be given the details about your second shot when you get your first shot. The second shot will be scheduled three weeks later, at the same location, the same day of the week, and the same time of day. |
